Ordering Section for Installations and Service

To all International Customers (U.S. and Overseas) sending shoe(s) for installation:
Instructions for completing CUSTOMS documentation
  • All paperwork for shoes being returned must be marked $1 in the value section
  • "Temporary Importation for Alteration" must be marked in the comments/remarks section

The Installation Tags below are used by BalancePlus to correctly install services to your existing footwear
Instructions:

1 Print this page
2 Fill out the required services for each tag
3 Cut out the tag
4 Punch a hole in the circle
5 Tie tags to the laces of the appropriate shoes
6 Include your detailed shipping information (delivery address), phone number and email address
7 Include your payment information (valid credit card# and expiry date) or advise if you would like to pay online.
8 Send your shoes to BalancePlus
